Out-of-pocket expenses are the costs you are responsible for paying when you receive medical care. These include amounts not fully covered by your insurance, such as deductibles, copays, and coinsurance. Your plan may also require you to pay for services that are not covered or for care received out of network. Once you reach your insurance plan’s out-of-pocket maximum, your insurance typically covers 100% of eligible in-network services for the rest of the year.

Deductible

The deductible is the amount you pay each year for your medical care before your insurance starts to help.

Example: If your deductible is $1,000, you pay that amount first. After that, your insurance begins to share the cost.

Copay

A copay (or copayment) is a set dollar amount you pay for a specific service, like a doctor visit or a prescription.

Example: You might pay $25 each time you see your doctor.

Coinsurance

Coinsurance is when you share the cost of care with your insurance after meeting your deductible.

Example: If your coinsurance is 20%, you pay 20% of the bill and your insurance pays 80%.

It is important that you contact your insurance provider (the phone number is located on the back of your insurance card) to understand your coverage and any potential out-of-pocket costs you may incur. We accept MOST insurance plans. Please submit a request for an appointment with complete insurance details, including a picture of the front and back of the insurance card, so we can determine insurance eligibility. **Please note: While we do all we can to confirm insurance benefits, insurance eligibility is not guaranteed until claims are fully processed. The patient or financial guarantor will be held fully responsible for any amount that is left after claims are processed.** Asthma, Asthma, Allergy & Immunology, PA retains the right to charge a $50.00 fee for any cancellation that occurs with less than 24 hours’ notice or a ‘no show’ who misses an appointment entirely without prior notice. The individual will be responsible for the payment of this fee, and it will not be billed to, nor paid by, the insurance company. We realize that last-minute things do come up, and we are sympathetic to unavoidable conflicts; thus, we evaluate late cancellations and no-shows on an individual basis. Please do not cancel your appointment unless it is absolutely necessary. Your well-being is a top priority, and we look forward to your next visit.
